Prescription Medication Compounding

Compounding is the process of combining, mixing, or altering the ingredients of a drug to create a medication tailored to the needs of an individual patient. This process typically involves two or more medications.

Sometimes, the health needs of a patient cannot be treated by typical over-the-counter (OTC) drugs. Additionally, compounded medications are recommended for those who:

  • Have an allergy to a certain dye or chemical in some OTC medications.
  • Are an elderly patient or a child who aren’t able to swallow a pill, or otherwise need a medicine in a liquid, gummie or more easily-delivered form.

The practice of compounded medication allows for custom-tailored medicine, designed with the individual in mind instead of a “one size fits all” mentality that most are used to.

Medication Therapy Management (MTM)

Medication Therapy Management seeks to optimize drug therapy and improve therapeutic outcomes for patients. MTM includes a broad range of professional activities, including but not limited to performing patient assessment and/or a comprehensive medication review, formulating a medication treatment plan, monitoring efficacy and safety of medication therapy, enhancing medication adherence through patient empowerment and education, and documenting and communicating MTM services to prescribers in order to maintain comprehensive patient care.

Medication therapy management includes five core components:

  1. Medication Therapy Review (MTR) – a systematic process of collecting patient and medication-related information which occurs during the pharmacist-patient encounter. This process also helps in the identification and prioritization of medication-related issues.
  2. Personal Medication Record (PMR) – includes all prescription and nonprescription products previously taken by the person, or slated to be taken in the future.
  3. Medication-Related Action Plan (MAP) – The MAP is a self-actionable plan necessary to achieve the patient’s specific health goals, as well as, to record actions and track progress towards their specific health goals.
  4. Intervention and/or Referral
  5. Documentation and Follow-Up

During the MTM session, medication-related problems are identified and an appropriate intervention resolution is decided. Following the patient intervention, the encounter is documented and an appropriate patient follow-up is scheduled.
